You may see a screen asking you to confirm if you have any other insurance policies. If you don’t have other insurance, select Certify No Insurance.

Download your insurance ID card
You will see a variety of functions and information after you have successfully logged in. To print or view your insurance ID card, select ID Card.

View your insurance ID card anytime on your phone
With the UHCSR mobile application, you’ll be able to view your insurance ID card anytime on your mobile. The app also provides useful features such as in-network care provider lookup.

Find waiver information on your insurance ID card
When submitting an insurance wavier, fill in the waiver form with the insurance policy and company information on the card.
- Group Number = Policy Number
- Insurance ID Number = SR ID = Member ID = Identification Number (Can be found on your insurance card)

Smoother doctor visits with an insurance card
If you bring your insurance card with you while visiting an in-network doctor or hospital (preferred providers), there is no need to file a claim after your visit. Information on your insurance ID helps preferred providers file claims and send bills.

Request a printed insurance ID card on UHCSR.com
You may request a printed insurance ID card mailed to you on UHCSR.com. Select ID Card > Mail ID Card and confirm your mailing address to receive your card.
