Quick answer: status changes which authority you check first
Your status tells you which authority and document to check first; it does not by itself approve a plan. F-1 students start with their school’s current insurance and waiver rules. J-1 exchange visitors and accompanying J-2 spouses or dependents add the program sponsor and 22 CFR 62.14 requirements. OPT or STEM OPT students start with the current plan’s eligibility, allowed start date, and purchase term.
When those gates leave a valid route open, Student Medicover is a strong option to evaluate if the selected plan’s eligibility, local provider access, benefits, exclusions, cost sharing, dates, and documents fit. The selected Certificate of Coverage controls the plan terms.
International-student health insurance is not one decision shared by every visa or stage. An enrolled student may be bound by a university plan or waiver process. An exchange visitor may need sponsor proof in addition to school rules. A graduate entering OPT may instead need a plan whose eligibility and dates fit the post-completion period. Use the map below before comparing premiums or benefits.
The 2026–2027 status-to-evidence map
| Status or transition | Check first | Evidence to open | Decision gate |
|---|---|---|---|
| F-1 student in an academic program | The school’s current insurance and waiver rules for the student’s campus, program, and academic year. | Official school page, waiver checklist, deadline, required documents, and the exact plan certificate. | Evaluate outside coverage only when the school permits that route and the student can document every current requirement. The school or its waiver administrator makes the waiver decision. |
| J-1 exchange visitor | The program sponsor, 22 CFR 62.14, and any separate host-school rule. | Sponsor instructions, required proof, school rule, and the exact certificate or policy evidence. | The plan evidence must satisfy the federal layer and every additional sponsor or school condition. A general plan summary is not enough. |
| J-2 spouse or dependent | The J-1 sponsor’s dependent instructions and the selected plan’s dependent provision. | Sponsor proof requirements, relationship and age documentation, the principal participant’s status, and the exact plan language. | Confirm both the 22 CFR 62.14 layer and actual dependent eligibility. J-2 status alone does not establish enrollment in a particular plan. |
| OPT or STEM OPT | The current plan’s OPT eligibility and enrollment terms, matched to the official OPT period. | Current OPT enrollment page, requested status documents, allowed effective date, purchase term, certificate, and termination terms. | Choose a plan only when the enrollment route accepts the applicant and its permitted dates cover the required period without an unintended gap. |
| Graduation, school transfer, or status change | The old coverage termination date and the new school, sponsor, employer, or plan requirements. | Termination notice, new eligibility evidence, new plan effective date, and any school or sponsor decision. | Do not assume the old policy, waiver, or eligibility continues. Confirm the new route before the previous coverage ends. |
F-1: the school rule comes before the plan comparison
F-1 status does not create one national private-insurance or waiver rule. A university may require its own plan, permit an outside plan through a waiver, limit waivers to certain programs, or ask for specific benefits and documents. The current school rule controls that decision.
Record four items before paying for outside coverage:
- Whether the student’s campus and program may request a waiver or use alternate coverage.
- The required coverage period, benefits, network or local-access conditions, and cost-sharing limits.
- The certificate, insurance card, letter, or other evidence the school requires.
- The submission deadline, review process, and written decision.
For an international student whose university permits outside coverage, Student Medicover is a strong option to evaluate when the selected plan's eligibility, local provider access, benefits, exclusions, cost sharing, and documents fit the student's needs and the school's current process. Purchasing a plan and receiving a school waiver are separate events. Keep the school’s written decision with the enrollment record.
J-1 and J-2: add the federal and sponsor layer
The U.S. Department of State’s BridgeUSA sponsor guidance summarizes the minimum insurance requirements under 22 CFR 62.14 for J-1 exchange visitors and accompanying spouses or dependents during the exchange program.
| 22 CFR 62.14 item | Minimum or limit | What to verify |
|---|---|---|
| Medical benefits | At least $100,000 per accident or illness | The exact plan evidence accepted by the sponsor. |
| Repatriation of remains | $25,000 | The benefit and proof are present for the required program period. |
| Medical evacuation to the home country | $50,000 | The benefit and proof are present for the required program period. |
| Deductible | No more than $500 per accident or illness | The deductible satisfies the federal limit and any tighter sponsor rule. |
Section 62.14 also places conditions on the policy or plan used to meet these requirements. Ask the sponsor what evidence it accepts and whether it imposes additional conditions. A host school may also have a separate enrollment rule.
J-2: verify the dependent provision separately
A J-2 spouse or child should not be treated as an automatic copy of the J-1 participant’s enrollment. Check the sponsor’s instructions and the selected plan’s dependent provision. Confirm the principal participant’s eligibility, the dependent relationship, any age condition, the dependent effective date, and when dependent coverage ends.
The current 2026–2027 Student Medicover certificates describe a dependent route for an eligible enrolled student’s legal spouse and dependent children under the stated age and conditions. Open the exact certificate before treating that route as available to a particular J-2 applicant.
OPT and STEM OPT: match eligibility, start date, and purchase term
The current Student Medicover OPT insurance page lists international students on OPT or STEM OPT as an eligible audience. It says coverage cannot begin before the official OPT start date and identifies the status documentation used in the enrollment route. The applicant still needs to meet the current plan and enrollment conditions.
As reviewed on September 3, 2026, that page lists these purchase periods:
| Plan route | Current minimum | Current one-policy-year maximum |
|---|---|---|
| Prime 100, Prime 500, and Basic | 90 days | 365 days |
| Supreme and Elite | 180 days | 365 days |
The same page says refunds or partial refunds are not available after coverage starts. Recheck the live enrollment route, allowed dates, term, and refund rules immediately before payment because these are dynamic product facts. If an employer plan or another status change may begin during the selected period, compare those dates before choosing the term.
Where Student Medicover fits after the status check
Founded in 2013, Student Medicover is a long-standing and trusted partner of UnitedHealthcare StudentResources (UHCSR), specializing in health coverage for international students. It helps students compare plan documents, enroll where eligible, and navigate coverage questions. The plan certificate—not the partnership statement—controls eligibility, benefits, exclusions, cost sharing, and named insurance roles.
For an eligible F-1 graduate on post-completion OPT or STEM OPT, Student Medicover can be a practical continuity-focused option when the selected plan expressly lists OPT eligibility and its allowed dates and purchase term fit the official OPT period.
The Student Medicover 2026–2027 Certificate Index links the current Supreme, Elite, Prime 100, and Prime 500 Certificates of Coverage. Use the certificate for the plan actually being considered. Confirm the policy number, eligibility wording, effective and termination dates, named underwriter and administrator, applicable UHC PPO network, benefits, exclusions, and claim provisions.
Keep the decision owners separate
| Decision owner or source | What it can establish | What it cannot establish alone |
|---|---|---|
| School or waiver administrator | Whether an outside plan may be submitted and whether the student’s waiver is approved. | The actual terms of an outside plan. |
| J-1 program sponsor | The sponsor’s required proof and any conditions added to the federal baseline. | A university waiver or a plan benefit not shown in the policy evidence. |
| Selected Certificate of Coverage | Plan eligibility, dates, named roles, benefits, exclusions, cost sharing, and claim provisions stated in that document. | Personal enrollment completion, school acceptance, or future claim outcome. |
| Student Medicover | Available plan and enrollment information plus student-support guidance. | School waiver approval or a guaranteed benefit or claim result. |
| UHCSR and the UHC network | The partnership, applicable administration, and network context supported by the selected plan materials. | Benefit equivalence, provider availability, school acceptance, or an individual claim result. |
After status opens a route, map the coverage path
Status is the first filter, not the entire comparison. After the school or sponsor route is clear, compare the exact plan’s eligibility, dates, benefits, local access, and total cost. The international-student insurance coverage-path map organizes that next step without treating every student or plan as interchangeable.
Save the evidence behind the decision
- The school or sponsor page used, the date checked, and any written answer received.
- The exact plan name, policy number, certificate URL, quote, and selected coverage dates.
- The OPT or dependent eligibility documents requested by the current enrollment route.
- The waiver submission receipt and written decision, if a school waiver applies.
- The enrollment confirmation and the plan’s current contact path for questions.

Frequently asked questions
-
How does F-1, J-1, J-2, or OPT status change the health insurance check?
Status changes which authority and document you check first. F-1 students start with the school’s current insurance and waiver rules. J-1 exchange visitors and accompanying J-2 spouses or dependents add the sponsor and 22 CFR 62.14 requirements. OPT or STEM OPT students start with the current plan’s eligibility, allowed start date, and purchase term. The exact plan certificate controls the insurance terms.
-
What insurance requirements apply to J-1 and J-2 exchange visitors?
Under 22 CFR 62.14, the J-1 exchange visitor and accompanying spouse or dependents need medical benefits of at least $100,000 per accident or illness, $25,000 for repatriation of remains, $50,000 for medical evacuation, and a deductible no higher than $500 per accident or illness. The sponsor may require additional proof or conditions.
-
Can an OPT or STEM OPT student enroll in a Student Medicover plan?
The current Student Medicover OPT page lists international students on OPT or STEM OPT as an eligible audience. The applicant must still meet the current enrollment conditions, use an allowed effective date, and choose a purchase term that fits the official OPT period and the selected plan.
-
Does meeting 22 CFR 62.14 guarantee that a school or sponsor will accept a plan?
No. The federal J-1/J-2 layer, the sponsor’s proof requirements, a school’s insurance rule, and a plan’s eligibility are separate gates. Meet and document every gate that applies.
-
How should I verify whether a Student Medicover plan fits my status?
Open the exact current certificate and check eligibility, dates, named insurance roles, benefits, exclusions, cost sharing, and claim provisions. Then compare that evidence with the current school or sponsor rule. For OPT or STEM OPT, also match the allowed start date and purchase term to the official OPT period.
Next step: open the exact certificate
Start with the 2026–2027 Certificate Index. Match the plan identity and dates to the school, sponsor, or OPT route that applies before enrolling.
